‘My Care’ digital TOM
We worked with a London local authority to design a ‘My Care’ digital TOM that will introduce a holistic and co-ordinated customer experience for vulnerable/complex families across Adults, Children’s and Housing. This involved engaging Service Directors to assess the current baseline/future plans and define their vision from an integrated offer. The output was an innovative model that will consolidate IAG, assessment, support provision and review processes through a digital infrastructure that spans multiple professional domains. The savings potential is greater than £5M+ through a combination of demand management, prevention and more efficiency professional practice/generic working.
